Passport Services
These passport services are available by appointment only:
- First time passport;
- Passport for minors;
- First time 10-year passport;
- Replacement of a lost, stolen, or damaged passport;
- Renewal of a passport issued more than 15 years ago;
- Renewal of a limited validity passport;
- Name changes without a court order, divorce decree, or marriage certificate (see here).
No appointment needed for:
- Most adult renewals. Click for instructions on how to renew by mail or online. If you are ineligible to renew by mail or online you will need an appointment. We cannot process or accept renewals done by form DS-82.
- Passport photos. Come in anytime for photos, or they can be taken during a passport appointment. $10 per set. Please note for online renewals the State Department has instructions here to take photos with your phone at home.

Passport forms are available here. You may fill out and print the online forms or stop by Town Hall prior to your appointment to pick up paper forms.
You must arrive to your appointment with a completed DS-11 form. If you arrive unprepared you will be asked to reschedule your appointment for a later time or date. Please also see the Passport Fees section below so you come prepared with the proper payment methods.

- Evidence of U.S. citizenship - Original/Certified Birth Certificate or Certificate of Naturalization (NO COPIES) more info;
- Valid Colorado Driver’s License, Instruction permit, or ID card more info;
- Previously issued passport book/card;
- Social Security number;
- One parent recommended with valid ID;
- Check or Money Order payable to the Department of State.
Minors and both parents must be present, and bring the following:
- Evidence of U.S. citizenship - Original/Certified Birth Certificate or Certificate of Naturalization (NO COPIES) more info;
- Both parents with valid ID;
- If both parents are not available, click here;
- Social Security number of minor applicant;
- Previously issued passport book/card;
- Check or Money Order paid to the Department of State.
Department of State Passport Fees: payable to the Department of State - check or money order ONLY.
- Adult Book- $130 per application;
- Adult Card - $30 per application;
- Minor Book - $100 per application;
- Minor Card - $15 per application;
- Expedited Fees - $60 per passport;
- Overnight Return Delivery - $21.36;
- File Search Fee - $150 - if you've had a U.S. passport in the past, and you cannot submit it with your application as evidence of U.S. citizenship.
Town of Gypsum Passport Services Fees: payable to the Town of Gypsum - cash, check, or credit card.
- Passport Processing Fee - $35 per passport;
- Passport Photos - $10 per photo.

Take I-70 West or East to Gypsum, CO. Take EXIT 140 and travel south, straight through the roundabout, continuing under the railroad tracks. At the traffic light turn right onto Valley Road. Go approximately 400 yards on Valley Road and turn left onto Lundgren Blvd., then turn left at the "T" and the Town Hall is the first building on your right.
